A "Must Add" to the document pouch for any type of evacuation whether wilderness, water or across the street away from the burning apartment complex, is the:

Like article mentions to take a picture of it, I also recommend to make a few photocopies of it. For about a month after shot/jab, one's immune system is weakened while building up the immunities. Thus, desirable to have a photocopy of this COVID doc visible on one's person such as in field jacket pocket. Emergency Responders might see the doc and understand their patient/victim has weakened immune system.

Per the article, if CDC is not collecting vaccine info on who got the shots/jas, then surely the collected info is kept by the USDA Commodity Crop Insurance regional office and the Consumer Product Safety Commission.

I have a small ziploc envelope Liquid Nails glued to inside brim of my evac hard hat. Haven't gotten the shot/jab yet ... waiting for the Johnson and Johnson vaccine to arrive and be available at a clean place that knows about and practices hygiene.....but when getting photocopies of my CDC COVID record, a copy in helmet envelope AND in jacket pocket.

Other stuff in document pouch will be a few 100,ooo yen notes, some twenties, some Benjamins, some tranquil art pictures, preselected family pictures, passport, birth certificate, military papers, skills certificates ... get stuck somewhere and authorities looking for a heliograph repair specialist ......

Per article, Crystal Tubbs clearly explained this CDC card also could be used for schools. Do consider taking that EMT course or the navigation one.

Don't forget a draft obituary and decent picture of yourself in case Prepper efforts were treated as a joke. Some will make it but most won't. Not too much new under the sun.
